It is required for all Amazon sellers, from what I understand, no matter what you sell.

It depends on your sales. I believe 10K a month hits the mark. However, as per TOS they can request it any time they want.

If they tell you that you need it, then you must have it.
The cost will vary depending on what you sell. I suspect your liability is not going to be extreme, but not zero.
(even booksellers are required to have liability insurance).
