FBA Inventory was mislabelled at the Amazon warehouse level and response has been very slow about fixing their error

One of our FBA ASINs is receiving returns for being an incorrect item to the point that an investigation has been launched. Prior to this we submitted pictures a customer had sent of the incorrect item they received in which the UPC on (our) manufacturer label and ASIN on the Amazon-applied label simply did not match one another. We were told to jump through several hoops which resulted in a bin check at the warehouse level (Case ID 16565132401 ) and we were sent images form the warehouse showing the completely wrong item being measured and told "it's right" and them closing the case. The item is incorrect because the spacing of the screw holes and shape is different and one of our other products submitted at the same time. Now we have received notice of an investigation with possible penalties (Case ID 16613396281 ) for incorrect items received. Please address this; the customers, the seller and FBA investigation team are saying the item is incorrect, we have photo evidence from three different sources and it is clearly the amazon warehouse's fault so is there any way to make action happen and establish some sort of accountability?

This is still an issue. Since making this posting we have had the warehouse send us two different products in images claiming bother were the "right product." I informed them that if their claim is that the item is correctly labeled on their end then we should not be liable for any returns of incorrect items. We have since had *11* customer returns for incorrect item. We were suggested to place a removal order. If we do this I do not believe we should have any removal order fees incurred since it was not our error and all of our products are labeled with appropriate UPC. I need response on this issue.
